Initial Comments:
An offsite follow up survey completed on October 12, 2021, found that Breathe Easy Independent Living Services, LLC. had corrected the deficiencies cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. The deficiencies were cited as a result of a State relicensesure survey and a complaint investigation survey completed August 9, 2021.
